TERMS & CONDITIONS FOR URGENT CARE & WELFARE CENTRE SERVICES
1. Purpose
These Terms & Conditions (“Terms”) govern the provision of urgent medical, welfare, and related support services (“Services”) by Rutland Medical Solutions Ltd. (“Centre,” “we,” “us,” or “our”). By using our Services, you (“Patient,” “Client,” or “you”) agree to these Terms.
2. Scope of Services
2.1 The Centre provides non-emergency urgent care, wellbeing treatments, preliminary assessments, and referrals.
2.2 The Centre does not provide comprehensive emergency services. If your condition is life-threatening, you must call the emergency services immediately.
2.3 We may refuse, postpone, or discontinue Services if your condition is outside our scope of practice or poses safety risks.
3. Eligibility & Registration
3.1 You must provide accurate identifying information and complete registration and medical history forms.
3.2 Patients under 16 must be accompanied by a parent/legal guardian.
3.3 The Centre will not accept patients under two years of age.
4. Medical Disclosure
4.1 You agree to provide complete and truthful information about your symptoms, medical history, medications, allergies, and any relevant conditions.
4.2 You must inform the Centre of any changes to your health during your visit.
5. Assessment & Clinical Decisions
5.1 Care decisions will be made by qualified healthcare professionals based on clinical judgment.
5.2 You acknowledge that the Centre may stabilise or treat minor to intermediate conditions and may refer or transfer you to a hospital or specialist when necessary.
6. Treatment Limitations
6.1 Diagnostic capabilities may be limited depending on the Centre’s available equipment and staff.
6.2 Treatment recommendations are based on the information you provide; incomplete information may affect outcomes.
7. Fees, Payment & Insurance
7.1 All fees must be paid at the time of booking unless an alternative arrangement is approved.
7.2 You are responsible for confirming insurance coverage. The Centre is not liable for claims denied by your insurer.
7.3 Additional charges may apply for diagnostic tests, treatments, supplies, or referrals after the initial consultation.
7.4 You are responsible for the cost of any medications when collected from the agreed pharmacy that are prescribed by the Centre.
8. Referrals & Follow-Up Care
8.1 The Centre may recommend follow-up care with external providers.
8.2 You are responsible for arranging and attending recommended follow-up appointments.
9. Privacy & Confidentiality
9.1 All personal, medical, and welfare records are handled in compliance with applicable data protection and health privacy laws (e.g., GDPR).
9.2 Information may be shared with other providers only for treatment, operational, or legal purposes, or with your consent except where law permits or requires disclosure.
10. Behaviour & Safety
10.1 Abusive, threatening, or unsafe behaviour may result in refusal of service.
10.2 Weapons, illicit drugs, and prohibited substances are not permitted on the premises.
11. Liability
11.1 To the maximum extent permitted by law, the Centre’s liability for any claim related to the Services is limited to the amount paid for those Services.
11.2 The Centre is not liable for complications arising from:

inaccurate or incomplete information provided by the patient

failure to seek recommended emergency care

failure to follow aftercare or referral instructions

pre-existing or undisclosed medical conditions
12. Amendments
The Centre may update these Terms at any time. The latest version will be available upon request and applies to all services moving forward.
13. Governing Law
These Terms are governed by the laws of United Kingdom. Any disputes shall be resolved in the courts of that jurisdiction.
PATIENT DISCLAIMER & WAIVER OF LIABILITY
By signing the booking form, you acknowledge and agree to the following:
1. Voluntary Consent for Treatment
I consent to receive medical assessment, urgent care services, welfare support, diagnostic procedures, and treatments provided by [Centre Name] and its healthcare professionals.
2. Understanding of Scope
I understand that this Centre provides urgent but non-emergency care, and that some conditions may require immediate hospital treatment or emergency services. I agree to follow recommendations to transfer or seek emergency care when advised.
3. Risks of Treatment
I understand that all medical treatments carry risks, which may include but are not limited to:

No medical outcome can be guaranteed.
4. Accuracy of Information
I confirm that I have provided complete, accurate, and truthful information regarding my condition, symptoms, medical history, medications, and allergies. I understand that withholding information may increase risks.
5. Responsibility for Follow-Up
I understand that I am responsible for following aftercare instructions, obtaining recommended follow-up care, and seeking emergency assistance if my condition worsens.
6. Privacy Acknowledgment
I understand that my information will be kept confidential in accordance with applicable privacy laws and may be shared only for treatment, operational, or legally required purposes.
7. Release & Waiver of Liability
To the fullest extent permitted by law, I release and discharge [Centre Name], its staff, healthcare professionals, volunteers, and affiliates from any liability for injury, complications, adverse outcomes, or damages arising from or related to the care I receive, except in cases of gross negligence or intentional misconduct.
8. Right to Withdraw Consent
I understand that I may withdraw my consent for treatment at any time, except when withholding treatment may result in serious harm or is restricted by law.
